Au laboratoire de Dr. Roddy Hiram situé à l'Institut de Cardiologie de Montréal, nous nous intéressons aux arythmies cardiaques.

La fibrillation auriculaire (FA) est la forme d'arythmie cardiaque la plus fréquemment diagnostiquée en clinique. La FA peut provoquer des complications graves telles que les caillots sanguins, les accidents vasculaires cérébraux ou la mort subite. Parmi les facteurs de risque de la FA, on peut citer le vieillissement, l'hypertension, l'obésité, la maladie cardiaque congénitale. La maladie cardiaque droite, qui se caractérise par une hypertrophie et une dilatation chronique du ventricule droit est aussi comptée parmi les facteurs de risques importants de la FA. D'autre part, l'inflammation est le dénominateur commun de nombreuses maladies chroniques associées à la FA. Pourtant les médicaments anti-inflammatoires disponibles sur le marché ne permettent pas de traiter et prévenir efficacement la FA. Ils sont de plus associés à des effets secondaires délétères. De nouvelles stratégies thérapeutiques sont donc nécessaires pour identifier les voies de signalisation inflammatoires impliquées dans la FA, afin de proposer des nouveaux traitements dans l'espoir d'améliorer la situation des patients souffrant de FA.

Nous avons récemment démontré, dans un modèle expérimental de maladie cardiaque droite provoquée par la monocrotaline chez le rat, que la maladie cardiaque droite chronique provoque des remodelages tissulaires arythmogènes augmentant l'incidence de la FA (Hiram, R et al., 2019. J Am Coll Card). En utilisant une molécule autacoïde promotrice de résolution, la Resolvine-D1, nous avons montré qu'un traitement pro-résolution quotidien pouvait atténuer le statut inflammatoire auriculaire et prévenir la FA dans la maladie cardiaque droite (Hiram, R et al., 2021. Cardiovasc Res.). 

Néanmoins, les mécanismes moléculaires et cellulaires sous-jacents qui orchestrent le Trio décadent impliquant l'inflammation, la maladie cardiaque droite et la FA demeurent non-élucidés. Nous proposons que ce « trialogue » physiopathologique constitue un paradigme arythmogène qu'il faut cibler spécifiquement pour prévenir et traiter la FA.

Afin d'approfondir notre raisonnement scientifique, nous avons tracé les axes de recherche suivants:

1. Description in vitro de l'activation de l'inflammation au niveau moléculaire et cellulaire suspecté de favoriser la mise en place du substrat arythmogène au niveau atrial.

2. Caractérisation détaillée du profil inflammatoire progressif et non-résolut qui provoque l'arythmo-génèse auriculaire dans un modèle de maladie cardiaque droite in vivo.

3. Identification et ciblage pharmacologique des voies de signalisations inflammatoires pro-fibrillatoires impliquées dans le déclenchement, le maintien et l'aggravation de la FA.

4. Compréhension de l'impact du vieillissement et des différences biologiques liées au sexe, ainsi que des facteurs environnementaux, dans le développement du statut inflammatoire arythmogéne atrial, notamment dans le contexte de la maladie cardiaque droite.

Ces travaux seront mené à bien grâce aux équipements de pointe dont nous disposons à l'Institut de Cardiologie de Montréal en matière d'échocardiographie, électrophysiologie cardiaque, histologie, cartographie optique cardiaque, immunohistochimie, biochimie, séquençage ARN et modélisation mathématique. Le réseau de chercheurs locaux et internationaux que nous avons établit au fil de nos années d'études constitue un atout pour mener à bien ces travaux en collaboration avec les meilleurs experts dans le domaine.

Nous favorisons et encourageons le recrutement et la formation d'étudiants et de stagiaires qui sont inspirés par nos objectifs de recherche, afin de les superviser jusqu'à l'obtention de leur diplôme et les soutenir tout au long de leur carrière scientifique.

Nous croyons que ces objectifs de recherche ont le potentiel de conduire à de nouvelles connaissances sur le role de l'inflammation dans la FA, dans le but ultime de proposer de nouvelles stratégies thérapeutiques ciblant des voies de signalisations inflammatoires spécifiquement impliquées dans la mise en place du substrat arythmogéne.
