Brian White-Guay est médecin de formation et détenteur de spécialités en santé publique et médecine familiale. Il a joint l’industrie pharmaceutique en 1984 comme directeur de recherche clinique auprès de la compagnie Ayerst à Montréal, puis de Merck Frosst en 1986. 

Suite à un transfert dans la division de recherche de Merck Research Laboratories (MRL) aux États‐Unis en 1988, il a travaillé dans les affaires règlementaires internationales et en 1992 a été nommé vice-président des affaires règlementaires européennes à Bruxelles, puis responsable des relations avec toutes les agences règlementaires internationales en 2002. Il a été élu correspondant étranger de l’Académie Nationale de Pharmacie de France en 2003.

De retour aux États-Unis en 2004 comme membre du comité exécutif de MRL et vice‐président responsable de l’ensemble des opérations mondiales de recherche clinique et des départements médicaux dans 50 pays. Il s’est joint par la suite à la Faculté de pharmacie de l’Université de Montréal de 2008-2018 à titre de professeur titulaire et a lancé avec succès le nouveau programme de baccalauréat en sciences biopharmaceutiques. 

Il agit depuis comme consultant en développement du médicament et est président du comité d'investissements du fonds de capital de risque en sciences de la vie AmorChem qu’il a joint en 2011. Il est également médecin-enseignant dans la Clinique universitaire de médecine familiale Notre‐Dame depuis 2009.
