Biographie
Dre Cécile Tremblay, est microbiologiste/infectiologue au Centre hospitalier de l’Université de Montréal (CHUM). Elle détient la Chaire Pfizer/Université de Montréal en recherche translationnelle sur le VIH et est professeure agrégée au département de microbiologie, infectiologie et immunologie à l’Université de Montréal. Grâce à deux cohortes pan-canadiennes qu’elle dirige, elle étudie les déterminants de la progression de la maladie VIH, ainsi que leur impact sur le phénotype de vieillissement accéléré associé au VIH.
Ses travaux de recherche portent également sur l’évolution génétique du virus et son effet sur l’entrée dans la cellule et la résistance aux antirétroviraux. Elle est directrice du Laboratoire de santé publique du Québec et œuvre au renforcement des capacités de recherche en santé dans les pays à ressources limitées.

Description
In the absence of treatment, human immunodeficiency virus (HIV) causes acquired immunodeficiency syndrome (AIDS), which has already killed more than 34 million people worldwide. To be able to spread, HIV needs first to enter human cells. To do so, HIV has developed a unique key (envelope glycoproteins, gp120 and gp41) that recognizes a particular lock (receptor CD4 and coreceptors CCR5 or CXCR4) in human cells (mainly lymphocytes T CD4 and macrophages). This key needs to move into the lock (conformational changes of gp120 and gp41) to allow viral entry. In the absence of no effective vaccines against HIV and with therapies aimed at HIV eradication remaining a far fetched goal, new ways to fight HIV infection are warranted. Our preliminary results indicate that antibodies recognizing this key in presence of the receptor CD4 are able to recruit cells from the immune system to eliminate HIV-1-infected cells through a mechanism known as Antibody-Dependent Cell-Mediated Cytotoxicity (ADCC). However, little is known about how this happens and how the virus tries to evade from this response. This proposal will advance our understanding of what could be a critical mechanism required for elimination of HIV-1-infected cells and has the potential to develop new inhibitors or immunogens against the key of HIV.

Evaluation of a tailored virtual intervention as an instrument of prevention and therapeutic support to improve the health of PLHIV and reduce comorbidity associated with HIV

Formerly considered a terminal illness, HIV infection is today classified as a chronic disease. The advent of a powerful antiretroviral therapy (ART) has contributed to reduce morbidity and mortality substantially among persons living with HIV (PLHIV). However, the long-term use of these drugs, the normal aging process and the presence of certain risk factors have been found in recent years to be associated with the appearance of metabolic disorders. To prevent the emergence and evolution of comorbid problems such as diabetes and cardiovascular disease, it is essential that PLHIV adopt health behaviours, namely, ceasing smoking, engaging in physical activity, and following a healthy diet.
Unfortunately, the literature suggests that smoking is actually considerably more prevalent among PLHIV than in the general population (50-70%) and that physical activity levels are lower among PLHIV than in the healthy population. The fact that PLHIV are at higher risk for cardiovascular disease and diabetes owing to ART and the aging process and that they present lower levels of health behaviours will place an acute stress burden on an already taxed healthcare system, particularly in the area of chronic illness care. In addition to adopting health behaviours, PLHIV must make a constant and perpetual effort to manage their antiretroviral drug intake. Treatment adherence among PLHIV has been the focus of a good deal of research in the past decade and warrants being pursued on an ongoing basis for a simple reason: less than two-thirds of PLHIV have been found to adhere optimally to treatment.
Against this background, we are proposing a clinical innovation in the domain of information and communications technologies that offers a new model of accessible care delivery for the PLHIV clientele across Canada. The purpose of this study is to evaluate the effectiveness of a tailored virtual intervention to support PLHIV in managing the demands inherent to their health condition.

Une étude expérimentale a été réalisée afin d’évaluer l’efficacité de l’application Web VIH-TAVIE* en temps réel, au moment, à l’endroit convenu et selon l’ampleur des besoins des personnes vivant avec le VIH sur leur comportement d’adhésion à leur thérapie antirétrovirale. Cette application présuppose que les participants aient accès à Internet et qu’ils soient disposés à l’utiliser pour recourir à une information de santé. Les participants sont assignés de façon aléatoire soit dans le groupe expérimental (application Web soutien en temps réel) ou dans le groupe contrôle (liste de divers sites Web). L’adhésion constitue l’indicateur principal. D’autres mesures intermédiaires ont permis de cerner le changement : sentiment d’auto efficacité, attitude liée à la prise de la médication, inconfort lié aux symptômes, empowerment lié au traitement, soutien social perçu. * VIH-TAVIE (VIH-Traitement, Assistance Virtuelle Infirmière et Enseignement)
